Please fill out these two forms neatly and with as much detail as possible, and bring them with you to your initial appointment. VERY IMPORTANT: Before your first appointment please go to the TREATMENTS section of this website and read the information on the many interventional treatments available for chronic pain, including the treatment or procedure suggested by your referring physician.

For your initial evaluation, it is very important that you bring the following:

Insurance Card and Picture I.D.
MRI Films and Typewritten report (if already done)
CT Scan Films and Typewritten report (if already done)
X-Ray Films and Typewritten report (if already done)
Results of any other diagnostic testing (Bone Scan, EMG)
Copies of note(s) from any physician involved in the diagnosis and treatment of your pain condition, including your primary care physician and specialist physicians (i.e. orthopedic surgeon, neurologist, or neurosurgeon).
Copies of prescriptions for any pain medications dating back at least two months.
